Create an automatic CMS hospital report.

Choose the care you are planning and as many as three hospital locations. The guide retrieves current CMS public reporting data, shows measures related to that care, and prepares questions for each hospital.

1. Describe the care

The report uses the care category to select relevant CMS measures. The optional detail appears in your questions but is not used to diagnose or recommend treatment.

2. Select the exact hospital locations

Search by hospital name, city, ZIP code, or six-digit CMS facility ID. Select one result in each box.

What is automated: CMS overall ratings, hospital characteristics, selected outcomes, infections, unplanned visits, patient experience, timely care, and maternal measures when available. Leapfrog, Joint Commission, U.S. News, Healthgrades, Castle Connolly, and Mayo Clinic HealthLocator remain linked for direct review because their information is not imported without an approved data feed.

How to read the CMS data signal

The signal summarizes available CMS measures related to the care selected. It is not a recommendation or a declaration that a hospital is safe or unsafe.

Green — Favorable signal
At least two relevant comparative measures are available, with no condition-specific measure worse than national.
Yellow — Review carefully
Condition-specific data are limited or one relevant measure is worse than national.
Red — Discuss reported concerns
Two or more independent condition-relevant measures are worse than national.
Gray — Insufficient data
No condition-specific CMS measure with a national comparison is available.

Missing or suppressed data never produce a green signal. The CMS overall star rating and general safety-group counts are shown as context, but neither changes the condition-specific color by itself. Every report explains why its signal was assigned.
